I've only heard it used literally.
If someone is behaving very formally in a casual situation, you could say, "Relax, you're not in the presence of royalty".

This can be used to talk about meeting someone famous (like a well-known musician) or someone who's famous in a certain field (like the top table tennis player in the country if you're into table tennis).

It's not something that people use often, but most people would understand what it means.
