"How do you do?" is a universal polite greeting. When someone says, "That's a fine how-do-you-do," it is meant to use irony for sarcasm.

You walk out to your car and it has a flat tire. "That's a fine..."
You leave your office and someone is having a fight right outside your door. "That's a fine..."
