Context would help but the phrase is frequently used in reference to a news report in which the writer/journalist/reporter refers to two or more sources of informaton; for example, "The accident, by most accounts, including the police report, was the fault of the bus driver".

An "account" is a description of some kind of incident or matter of fact. Using "by most accounts" means that the description you are giving agrees with most of the others.
