I often hear this phrase...but I'm still not sure the meaning of it.
Can anyone clarify it?

Basically it means there is no other place on earth like home. You would say "home sweet home" when you come back home after a long trip and you know how good it feels to be back again, you are really happy and it gives you that special feeling to be on your home "turf" again.
This can also mean your country, your state, your town, etc. something close to your heart and very special.
