Default lesson plan

Hello! Can you please supply me with a lesson plan sample and what it must cover in general. For instance, if I am going to teach the simple present of the verb to be and its affirmative, negative and interrogative forms, how could I plan this skill? Thanks deeply.
Truly Yours;
Khalid
