3.2.

12. Ann ... me her address but I'm afraid I ... it. (give, lose)

The answer:
gave ... lost or have lost/ 've lost.

What troubles me:
For me have lost is the only variant answer here. They are chatting now. "I'm afraid" underlines that he now doesn't have the address.

Could you comment on why did they mention past simple for 'lose' here as the primary variant?

Present perfect is my variant. But the answer key is more inclined towards past simple. Why?

Both are possible. It's just another example of a poor question.

Ann gave me her address but I'm afraid I lost it.
Ann gave me her address but I'm afraid I've/I have lost it.

I can discern no difference in meaning between these variants.

Stop worrying about it and move on. :)

My feeling here is that the past simple is not meant to be presented as the 'primary' variant, but that it just happens to be the variant mentioned first, perhaps just because it looks better on the page that way round.
