filling in an application

Hello everybody!

Suppose that I am filling in an application. All of a sudden, I come across a blank space where my personal data should be typed in.

Should it be typed "name and surname" or "first name and last name"?

What is your opinion?
 
We don't use the word "surname" much in American English. Forms usually say "Last name/First name" or "Name (Last/First)".
 
I'd just write:

Name: Piscean Elihue Aloysius MOUNTBATTEN

or: MOUNTBATTEN, Pisecean Elihue Aloysius

I believe you are supposed to spell your name correctly on your CV- it impresses bosses no end. :-D
 
Is that your other middle name? ;-)
 
If it is, it's nuts.
