Depending on what it's the password to, all except 'unmindful' are possible.

Any sentence can be used either Matthew's or mine, right?

"Remiss" isn't really a common word, outside of some phrases like "I would be remiss if I did not..."

"It is very careless of me to forget the password" and "it is very careless of me to have forgotten the password. Do both the sentences have same meaning?

Yes, with the changes I've made above.
 
I think the following comment about the perfect infinitive used in a past tense sentence applies to the OP's sentence.
'to have followed is, I suppose, technically wrong. As with my example above, few people will be bothered by it.

Okay, if we are using past tense then only "have forgotten" is applicable, is it correct?
 
Not necessarily. You can say "It was very remiss/careless of me to forget my password".

Okay, "have forgotten" can be used with the present tense as well, is this what you are saying? But according to Goestation's reply it can be used with past tense only.
 
Where did GoesStation say that? If you read post #7 again, you will see that GoesStation indicated that "It was very careless of me to forget the password" and "It was very careless of me to have forgotten the password" are both correct.
