"Many a man/More than one man fell, didn't they? 'men' " but I also think using a singular pronoun is correct but it seems odd, silted or very formal to me.
Now, let's proceed to answer the fifth question.
Many a man gets confused about subject/verb agreement nowadays. The singular doesn't feel the least bit stilted or formal to me. The plural would just feel wrong.
