Which sentence is correct?

1) Covid-19 wake up, all must take care.

2) Covid-19 wakes up, all must take care.

A disease does not wake up. Do you mean it is prevalent?
Do not use a comma to join two independent clauses.

Are you telling people to wake up? Then say:


Covid-19! Wake up! All must take care.


2) Covid-19 wakes up, all must take care.

Do you mean that Covid-19 is active now? Then say:

Covid-19 is spreading. All must take care.
It's not clear what you mean. Can you tell us more about what you're trying to say?
 
Or possibly:

Covid-19 is a wake-up call- we must all take care.

We do need more- it could refer to the emergence of the virus.
