I stand corrected. Roman numerals are evidently preferred, at least by the NHS.

Maybe you don't use them in such context in the US? It's a British website.
 
The NHS can use them if it wants to — the rest of us are under no obligation to do so, and it can't be denied that many would read 'stage II' as 'stage 11'.
 
American authorities also seem to prefer Roman numerals.
 
I used "Stage 2" in post #2 only because of the potential confusion for some learners who might not be familiar with Roman numerals. However, the medical notes of all my relatives who have had cancer used Roman numerals throughout.

My grandmother was diagnosed with Stage II colon cancer in 1997.
My aunt was diagnosed with Stage IV lung cancer in 2002.
My mother was diagnosed with Stage II lung cancer in 2012.
 
My brother had stage IV with a different cancer- it seems fairly standard in the UK.
