In order to have it done a person should be younger than 30 years of age or should not be older than 30 years.

You've used the plural ages. Is this a surgery that can be done at only certain ages? For example, it can be done at 23, 27, or 31, but not at any other ages?

Do you really mean "or"?

Both sentences are grammatical. Number one means what I think you want to say and uses a minimum of words to say it — well done. Number two expresses your opinion of the patient's age. That's not what you mean.

Because if you start reading, your English will probably improve. Without reading, you can only progress very slowly.
 
I heartily agree with GoesStation on this. I have a suggestion - why don't you take three or four weeks away from this forum and spend the time reading anything you can get your hands on in English? Read news websites. Read simple articles online. Find some "easy reader" books. Read children's books.
 
Your two options are slightly different in meaning. The first says must be under 30, so a 30 year old would be excluded. The second includes the 30 year old.
 
One possibility. Say:

To have the surgery you need to be thirty or younger.
