I have requirement to to execute a script in my database, but before that I need
my onsite Manager's approval for that.

How should I ask him for approval. Can some one suggest ?

Justin - Could you provide your approval to execute the script attached.

Is that correct?

Can someone suggest other options.

This is really important. If something goes wrong in my english he is going to bang me.

He is an native speaker (American) . I am an Indian .He always finds faults in my english.

Remember that English starts with a capital letter, in common with Hindi, Telugu, French etcetera. Do you mean that he is always criticising your English?

I think he should be more polite. It might help if you took more care over your typing and remembered to start "English" with a capital letter.
