L. G. Alexander says in his LONGMAN ENGLISH GRAMMAR:

Who is to blame/to be blamed?

Do the two constructions mean the same thing? Thanks a lot.

He was blamed = People said he was responsible (but this may be incorrect)
He was to blame = he was definitely responsible

Could you tell me further what is the difference between 'Who is to blame?' and 'Who is to be blamed?'
