It was the third time that he won/had won the game.

I think "had won" is correct, but is "won" possible?
 
"That was the last time that I saw her."── quoted from my Oxford dictionary.

Why is "saw" used instead of "had seen" here?

The following is the context.

To talk about the first or the last time you do something, use the first/last time (that) I ... : This is the first time (that) I've been to London. ◇ [STRIKE]This is the first time for me to go to London.[/STRIKE] ◇ That was the last time (that) I saw her.
 
I keep thinking you can only win a game once.

It would make more sense to me as 'It was the third time that he had won the championship.'
