Don't say that out loud! I can say:

What if she understands you.

VS

I'm pretty sure she understands you.

Are both okay?

It's not clear what you're asking, Batman.

Please rephrase it.

What does "Don't say that out loud!" have to do with anything?

My friend calls an Asian girl fat behind her back. First I say: Don't say that out loud! Then, I can say:

What if she understands you.

VS

I'm pretty sure she understands you.

Are both okay?

It would be more natural to say "Don't say that out loud. She might hear you". Mind you, I would probably say "Don't be so rude!"

But she was right in front of us, and gave us a nasty look, so I say:

I'm pretty sure she understands you.

Can be said?

You could say "I'm pretty sure she understood you" although the present tense isn't completely wrong. I would still suggest to your friend that he/she not be so rude about people in future.
