Which of the following is correct?

1) To visit your doctor to have you checked
2) To visit your doctor to have yourself checked

If both are possible, do they have the same meaning? If not, when should I use each of them? Would you please give me some examples?

Thank you.

Neither one is a complete sentence.

If you remove the initial 'to', the second one is possible as an imperative sentence.
