Evaluation of the patient's history, laboratory data, diagnostic tests, radiological studies, and treatment protocol ( indicates indicate ) little improvement in his overall condition.

Which word should I use and what is the rule? I chose indicates. Is that correct?

Which word should I use and what is the rule? I chose indicates. Is that correct?

Evaluation is single, so the verb has to be indicates.
