Is the usage of by correct here?


Stress is often called a 21st century illness but it has always been with us if perhaps by different names.
 
I would use "with".
 
Are there the other possibilities?
 
Why would you need alternatives?
 
Good answer. I can't think of another preposition to use there.
 
The sentence is fine as it is, isn't it?

It can be interpreted as:

Stress is often called a 21st century illness, but it has always been with us if [it has] perhaps [been called] by different names.

Those words in square brackets are omitted.
 
In your restatement, I would use "even" before "if" and delete "perhaps".
