My friend Steve told me he got vaccinated already. Here the government tells people who get vaccinated that the vaccinate is not 100% safe (can't provide 100% safe protection). One still has a chance to get infected with the virus. So I asked still if he was told something like this when he got vaccinated. I asked:

Were you told that the vaccinate is not 100% safe?

Is the question natural? Or how can I ask the question?

No, safe is not the right word because that suggests that it will have adverse effects. You mean effective. The verb is vaccinate, and the thing that goes into your body is the vaccine.
