Doctor: Morning. Have a seat, please. What's the problem?
Patient: My stomach hurts.
Doctor: When did you start to feel like this?
Patient: This morning. Maybe I had too much ice cream last night.

Is it also OK just to ask “When did that start?” instead of “When did you start to feel like this? ” I don’t think the original sentence is very common.

"When did that start?" is perfectly natural. "How long have you had that problem?" would also work. "How long have you been feeling that way?" would be good too.
