Options 5) and 6) are not asking a question. The rest are asking for permission/approval.

15. I'd like to have a picture (taken) with you. Is that OK with you? The double "with you" sounds awkward. Change the second part to "Would that be okay?" and it's okay.
16. I'd like a picture (taken) with you. Is that OK with you?This is the only one I wouldn't use "taken" in. Same as above - "Would that be okay/possible?"
