In BrE, the stress on the verb "fragment" definitely comes on the second syllable. In the nouns "fragment" and "fragmentation", it's on the first.

This is what's specifically being asked:

I think I wasn't clear enough in my question. I'm looking for words that have pairs: a noun with a stress on the first syllable and a verb with also a stress on the first syllable, the difference between them being a single vowel shift

Words with a similar pronunciation pattern to "document" include "instrument" and "element." Like "document," these words have different stress and vowel patterns depending on their noun or verb forms.
1- Please correct the info on your profile.

3- The words "element" and "instrument" are not verbs.
