She wants in the letterhead.
She wants on the letterhead.

The first two do not make sense.

'...on the website.'

Informally, one could ask to be on the letterhead of a company.

I'm sure one could but that doesn't make "She wants on the letterhead" correct. It would have to be "She wants to be named on the letterhead/She wants her name to be on the letterhead".

Usually, but not always. In the current case "to be" is understood.
