Which of the following test problems is correctly worded? (The given answer is C) And can the verb 'urged' be used with 'attacks' as its subject? Thank you in advance.

Not quite sure I understand your question, but what I think the sentence is trying to get at is that an attack prompted an international response. At least, this would be the meaning of the second sentence with answer C injected. The first sentence doesn't make sense.
 
How about

The Somali pirates' frequent attacks at sea led the United Nations to ______ all nations to take immediate action.

I suspect it should be like that:

The Somali robbers' frequent attacks on the sea urged the United Nations to call on all nations to take immediate action.


urge (verb) = to strongly suggest that someone does something
