The past tense of hurt is also hurt, not hurted.

My head hurts. (Present tense.)
My head hurt. (Past simple tense.)
My head has hurt. (Present perfect.) NB - Rather unnatural and would be followed by a time identifier ie "My head has hurt every day for the last three weeks".
My head had hurt. (Past perfect.) Same note as the last one.

I would assume you meant that you have/had a headache. If your head was actually injured/damaged, you would use "My head was hurt in an accident" or similar.

I want the exactly meaning of two sentences. You explain that the sentence meaning for me.
But, I want to know if I said that "My head was hurt recently" or "three days ago". Is it correct?
