The second should be "Has she played piano?"

The last sentence is not a proper question. It should be "How do I ask whether she has learned to play piano?"

Why the verb is in past tense?
She could be still learning piano and not yet complete the lesson.

I took my lead from your sentences. It could be "How do I ask if she is learning to play the piano?" My comment was made about the correct form of a question.

On the subject of the correct form of a question, jasonree, 'Why the verb is in past tense?' should be 'Why is the verb in the past tense?'

(In BE, most people say 'Does she play the piano'.)

The difference should be obvious, but apparently needs to be stated.

A native English speaker and VIP member with over 5,000 posts, such as James, can respond as soon as he likes.

An ESL student with less than 40 posts should wait more than seven minutes before hazarding a reply.

There is no strict rule but if you are going to reply to another learner's question, you need to be pretty sure you are giving a helpful/correct response. In the response you gave, you did not leave a space between "[STRIKE]plays[/STRIKE]" and "played" and you didn't leave a space between "[STRIKE]learns[/STRIKE]" and "learned". You did not correct the question construction. "How to ask ..." is not the correct way to ask a question in English but you did not correct it to "How should I/How do we ...?"

If your corrections aren't correct, the OP will become very confused.
