Does "which opened into the entrance hall" refers to a habit or something else is intended? I mean, does the door opened in that occasion?
Does "since" mean "since his death"?

The Rev. G. Vale Owen describes* the return of George Leaf, one of his Bible Class lads in Orford, Warrington, who joined the R.F.A. and was killed in the Great War. Some weeks later his mother was tidying up the hearth in the sittingroom. She was on her knees before the grate when she felt an impulse to turn round and look at the door which opened into the entrance hall. Also that smile of love told her that his heart was still with the old folks at home. She is a sensible woman and I did not doubt her story for a moment. As a matter of fact, since his death he had been seen in Orford Church, which he used to attend, and has been seen in various places since.

No, it has nothing to do someone actually opening the door. If a door opens into a particular room, it simply means that if you go through that door, that room is where you end up. So the door from your hallway to your kitchen "opens into the kitchen".

The first "since" refers to his being seen in Orford Church after he died. The second "since" means that after being seen in Orford Church, he was seen in various (other) places.
