Dear Professor XXXXX,

Last time, I was abroad during your visit, so I did not have a chance to meet with you.

Thank you for taking the time to write on my behalf for my graduate school application. I appreciate your support throughout this process. I have submitted your contact information online to the graduate admissions office, and SF State should have emailed you already.

I will keep you updated about my progress in applying to graduate school. Thanks again for your assistance. It is much appreciated.

If you would like any further information, you are welcome to contact me.

Yours sincerely,

XXXXXX

That's the only line I don't like. Opening with "Last time" is confusing because it doesn't refer back to anything. I would start with something like "Unfortunately, I was abroad the last time you visited so I didn't have the chance to meet [up] with you".
