I have a question. I have the sentence:

Has anyone ever hypnotized you?

I am supposed to make this sentence passive.

Have you ever been hypnotized?

Why do I use has in the first sentence and have in the second? Also, what verb tenses are these sentences.

Thank you for your help.

Why do I use has in the first sentence and have in the second?
Because has refers to anyone and have refers to you.
These sentences are in Present Perfect.
