Can anyone tell me which sentence is correct?

It depends on what is above. If you are referring to answers in a test, 'are' is correct. If you are referring to information, 'is' is correct.

Why not be specific? - I certify that the above information is correct.

If this is on a legal document, I suggest that you consult a lawyer about the exact wording that is required.
