[General] Any recommendation for how to start study English?

I arrived in The United State as an adult and lived for 17 years. Now I'm in my 40s. I speak English but not fluently. And I'd like to improve my English in read/write/speaking. My ultimate goal is to read online article/novels and speak English fluently.

First of all, let me tell you how much I know and how much I can understand.

1. Grammar - Absolutely no experience. I know what is noun/verb/adjective... but absolutely nothing else.

2. Vocabulary - I would like to say.. it's below intermediate. I'm facing 5~10 words that I don't know when I read Yahoo news. Sometimes more than that. I probably know about 5~10% words from TOEFL 500 words.

3. Listening - I've been watching Netflix for couple of years. I could understand about 80~90% from "Louie" "How I met your mother" "Desperate Housewives" but couldn't understand about 40% of expression/words from "Revenge" which I just finished watching season 4. Especially when Conrad/Victoria/Nolan speaks.

I don't know if these information is enough for you to judge my English level but I think it might give you a little idea.
I honestly don't know how to and where to start. Any recommendation of how to start study? Any suggestion of Books/Novel/Website?

Wherever you are in the USA, there's certain to be a school or college nearby running courses for people like you.

The standard of your writing is not bad at all. Here are a few corrections:

Any grammar/vocabulary book suggestion? Unfortunately, I'm not able to attend any of English class at this moment.
