"On the move" means to be going pr travelling. I think the writer is using it incorrectly. I would use "underway" or "up and running" here. "Back on the move" means "back going" and it's kind of strange. What do you think?

Well, does "on the move" mean "underway" in this case? The dictionary defines it as "going somewhere", that's why it sounds weird.
 
From the limited context I can see in the tweet, it looks as if play was temporarily stopped while a substitution was made. The match has now restarted (it's "back on the move") with one change (substitution) for the away team (the visitors). Walker has come off (has stopped playing) and Politico has come on in his place.

Edit: I just noticed the time stamp of the match - 46'. That's the 46th minute of the match. Each half is 45 minutes so I actually now think that play has restarted after the half-time break. Walker played the first half but Politico is playing the second half instead of Walker.
