If there is a picture hanging on the wall, it's above the bed, should I say "The bed is under the picture" or "The bed is below the picture"?

Either way is fine.
 
I would use "below". The bed is under the ceiling, not under the picture.
 
Other options include beneath and underneath:

The bed is beneath the picture.
The bed is underneath the picture.
 
If there is a picture hanging on the wall no comma here it's above the bed, should I say ...
Note my corrections above. You haven't used a particularly natural context for this particular post because we'd rarely describe where the bed is in relation to the picture, but the other way round. We'd say "There's a painting [on the wall] above/over the bed".
 
The prepositions 'above' and 'below' work best for describing relations like this, so if the picture is above the bed, then the bed is below the picture.
