Do we make "changes to" something or "changes on" something or "changes in" something?

What is the correct expression? I always say "I have made changes to something". Is it always followed by "to"?

Either 'changes to' or 'changes in', depending on context.

I've made changes to the agenda.
We will see changes in the new administration soon.
