Normally we use the word " need "without ' to infinitive ' but in many cases there is " to + infinitive

This is confusing , isn't there any hard and fast rule ?

He needs to lose a bit of weight.
i need to do some shopping .

I don't think we need ask him.

I also read there is no -s or -es with singular subject when it comes with "the word Need'

Dare and need are marginal modals. It means in non-assertive territories (interrogatives, negatives) they can behave in two ways:

1. like a modal auxiliary: Need you go home?
2. like a (normal) full verb: Does she need to go home?

In assertive territories, the main verb construction is used: He needs to go home.
Blend of the two constructions is widely used for 'dare' :

I do not dare go home.

Yeah, the main verb construction is much more common, especially in the US.
