2) Did she not recognize me, or did she ignore me?

Are both correct?
Are they interchangeable?
Is there any difference between the meanings?
 
They're both grammatically correct and there's no difference in meaning. I wouldn't use the comma.
 
Perhaps:

Did she fail to recognize me, or did she simply ignore me?
