I haven’t seen my middle school classmate who’s also an old friend of mine for a long time. I saw him the other day. I noticed that his eyelips are double now. They used to be single. I asked him:

Did you do the cosmetic surgery?

Is the italic sentence natural? The intended meaning is “Did you have your eyelips doubled?”​

Note the above corrections:
1- Past perfect.
2 "eyelid".

Did Have you do the had cosmetic eyelid surgery?
3- Present perfect (the surgery happened in the past, but it's relevant to the present conversation).

Note also that you'd it may come across as tactless asking someone a personal question like that. It depends on how close you two are/were as friends.
