Hello, I am writing a business email. However, I am not sure I should use "is" or "to be" in the following sentence. Please advise. Thanks, P

"This information is missing in the final close-out documentation package. Please follow up with the Supplier to ensure this deficiency is addressed immediately prior to final release of holdbacks."

Only 'is' is grammatical.
 
Why capitalize "supplier"?
 
Mayltthew,, the word is "ensure" and the wording should be:

Please follow up with the supplier to ensure that this deficiency is addressed prior to final release of holdbacks.
