It seems an odd way to say it. I would say "I'd like to exchange this shirt".
 
This may just be American. It's a common thing to say. "Hi, I have a return." "Can I help you with a return or an exchange?" "Hi, this is just an even exchange, not a return. I'd like to get this in a large instead of an extra-large."
 
I can back up Barb D's claim. I've heard 'exchange' used liked that before, though 'return' is a bit more common. It could just be a US thing.
