Do I need to use 'follow up with', 'follow up on' or 'follow up' in the above sentence?

I'm surprised to see "Under some circumstance" (singular). That would be "Under some circumstances" in BrE.

Which preposition you use depends on what you mean.

You can use with to suggest that the patient and the physician meet in person. You can use on when the physician follows up without meeting the patient in person.
