Hello everyone.

Furthermore, there is a childlike enthusiasm to the high-hope person’s approach to what he or she is doing. As an example, consider Carl, a seventy-two-year-old retired electrician, who spends four afternoons each week volunteering at the hospital. “It’s always different here,” said Carl. “I mean I get to meet so many nice people from all over. And my time spent here helps people, and me… . Why, it’s like I’m a big kid.”

Does "from all over" mean "from all over the country"?
 
I doubt that the hospital draws patients from all over the country. More likely it means all over the region or area.
 
It may also well be the case that even some of the people who live in that area are immigrants and emigrants to the region, especially if this hospital is in a larger city.

Sometimes you also get people traveling for specialized treatment, as well as the occasional tourist who needs medical attention too.
 
It depends on how many hospitals there are distributed in the country. Obviously, people go to hospitals whose locations suit their convenience.
