Hi guys,

Can we say "He fell down his headfirst/ shoulderfirst/bellyfirst"?
 
I am not a teacher.

If you separate headfirst (and the others) into two words and remove 'his', they will be OK.

'He fell down head first/shoulder first/belly first'

Falling down belly first, would be difficult. :-D
 
Maybe not for Santa Clause.

Baseball players might slide "head first" but I wouldn't say someone fell that way.

I'd say "he fell on his head."
 
It's possible to fall head first into something.
