"How long are you here for?" I would understand it as "How long are you going to stay here for?"

The reason I'm asking is that I heard it in a slightly different situation. A young doctor is working at a small clinic in Latin America. It's been about a month since she's been on the staff. A patient comes over to ask "How long are you here for?" "Not too long. About a month or so." Did she mean "How long have you been wroking here for?"

Do we really need "for" in those sentences?

"How long are you here for?" I would understand it as "How long are you going to stay here for?"
That is correct. Your doctor was half way through her stay.

That's a totally different question, as you well know, Ostap.
