How to write a questionnaire

Salam Alikoum
I need to write a questionnaire of about more than 10 and less than 15 questions;I have no idea what the questionnaire should be about but the point is to write one and see if we are fellowing the right steps,yet I have no idea how to start one,could you help me pleaaaaaase!
