Is the following sentence correct and natural?

I decided to go back my hometown to meet my family.
 
You need a "to" after "back".

I'd also write "home town", as two words. Some might prefer a single word however. (This might be a BE-AmE difference. "Hometown" sounds AmE to me.)

Other than that, it sounds fine.

"Meet" doesn't seem to be the right word. Unless this is family that you have never met before.

If you're talking about going back to see Mom and Dad, then I would say that you went to see them. Or visit them.
 
I write hometown as one word.
 
I'd write "hometown" too. It does sound odd with "meet". You could, however, use "meet up with". I'd probably use "visit".
