[Grammar] I didn't know what was my fault.

Dear all,

Can you please tell me which of the following sentences are correct? I think the auxiliary verb must be in the end of the sentences but I'm not sure.Maybe both version are correct.If I'm right about the place of auxiliary verb must I use 'are' after 'correct' in my question sentence?

1) I didn't know what was my fault. or I didn't know what my fault was.
2) Can you tell me where is my wallet? or Can you tell me where my wallet is?

No, that would be incorrect.

I didn't know what was my fault.
Incorrect.

I didn't know what my fault was.
Correct.

Can you tell me where is my wallet?
Incorrect.

Can you tell me where my wallet is?
Correct.
 
As far as "I didn't know what was my fault" goes, there is a scenario in which it could be used.

My teacher said to me, "It's your fault". The problem was I didn't know what was my fault.
 
Another scenario involves knowing the faults, or the problem. "We both made mistakes. I didn't know what was my fault, and what was Peter's."
