Is it correct to say, Kentucky, USA?

I've always been told to use the before USA. Is the following correct:
In the year 1963, Johnny Depp and American Actor and Producer was born in Kentucky, USA

Are addresses an exception?

There are situations where you can use "USA" without "the". This is one.

The sentence is incorrectly written by the way.
 
In (the year) 1963, Johnny Depp, and (the) American actor and producer, was born in Kentucky, USA.

The words in brackets are optional.
