You use the expression when receive something invaluable in return of a risky/painful effort; as:
Is it worth for that long and tiring climb? Of course, it was a long climb to the top of the hill, but it was worth it for the nice view from the top.

Risking your life and saving a child from being drowned has not been easy, but he says it's worth it: "Seeing the child’s smile is more powerful than you can imagine."

You can express the same meaning as: Is it woth the trouble/effort?

(1) Is it worth it? = Is some action justified?

(2) Tom lives in the city. Rents are very high. He has a chance

to get an apartment in the countryside for a very, very low rent.

But he will have to drive 50 miles each day to and from the countryside.

Is it worth it? Does paying a very low rent in the countryside

justify driving 50 miles to and from work every day? Some friends will

say, "Tom, it's worth it." Others will say, "Tom, it's not worth it."
