It puzzles me that how people who only minimally took care of their elders spend fortunes to push their death a few weeks with them going through a lot of pain and suffering and when death is confirmed sooner or later.

"their" in the bold letters could mean "people" or "their elders". Please tell me how I can make sure that it means the latter.

It's me, emsrd2. I framed it, but thought whether it could be rephrased better. I repeatedly find such errors in my sentence construction. So it helps me frame long sentences better from next time. Please correct it.
The intended meaning is that people don't look after their elders well when they are functional, but when death is hanging over them and are hospitalised, those people will spend fortunes that'll make them heavily indebted.

The best I can do is something like:

It puzzles me that people who have previously paid little attention to their older relatives suddenly feel the need to spend a fortune on delaying the death of those relatives once they have become old and infirm. They're going to die anyway - delaying it just causes more pain and suffering.
